Well, I looked at a CDOS disk with RDOS and as far as I can see, a
5 1/4" disk formatted as double-sided, double density (like yours) has:

Track 0, Side 0: SD, 128 bytes/sec, 18 sectors, no interleave (1-2-3 etc.)
Track 0, Side 1 and all subsequent: DD, 512 by/sec, 10 sec, interleave 4 
(1-5-9-3 &c)

Non-standard enough for ya? Standard for Cromemco, who always did like to 
do things a little differently :-\

Note that density/sides are format options, so a CDOS disk could be SS/SD,
SS/DD,DS/SD or DS/DD (except for track 0, side 0, which is always SD)

Hope it helps (BTW, didya get that disk copied to 8" OK?)
