I live in the UK, and tend to only buy stuff from eBay in the UK.  I did 
have a video camera shipped to me in its original soft-ish carrying case 
(bendy but self-supporting PVC cover, with thick padding inside), which 
was then packed tightly into a thick (about 1/2"!) cardboard box that 
fitted so tightly I had to split the box to get the case out.
If something is big and fragile, I just go and pick it up.  Worst case 
it's a 1,200 mile round trip.
